FRAILTY: ASSESSMENT & MANAGEMENT

A practical, person-centred, pathway to promote early identification and tailored intervention for frailty, sarcopenia, and malnutrition in older adults. Developed in collaboration with leading clinicians in the fields of malnutrition and frailty, this pathway integrates validated tools and aligns with best practice in Comprehensive Geriatric Assessment. It emphasises the central role nutrition and muscle strength play in frailty progression and highlights opportunities for targeted, modifiable interventions to improve health and wellbeing.

Frailty, sarcopenia and malnutrition guide

This guide focuses on frailty and the syndromes sarcopenia and malnutrition, which often co-exist and contribute to frailty. It aims to enable health and care professionals to recognise the signs of frailty, sarcopenia and malnutrition, and encourages actions to identify and tackle nutritional issues that are contributing and could be addressed, with referral for further assessment if required.
